Detailed introduction

SAA is a polymorphic protein family encoded by multiple genes and acts as a precursor to tissue amyloid A. It belongs to acute-phase reaction proteins. SAA is mainly produced by hepatocytes. The SAA family includes acute-phase SAAs (A-SAAs) and constitutive SAAs (C-SAAs). A-SAAs are major acute-phase proteins, with concentrations increasing up to 1,000-fold during inflammation. In contrast, C-SAAs are found only in humans and rodents and are rarely induced. Upon inflammatory stimulation, SAA levels rise rapidly, peaking within 8–12 hours, with amplitudes up to 2,000 times the normal value. Its half-life is short (50 minutes), allowing rapid normalization post-inflammation. SAA increases in both viral and bacterial infections, whereas CRP remains low in viral infections. SAA is more sensitive than CRP for detecting mild inflammation, making it a valuable marker for early-stage bacterial or viral infections in pediatric patients. Early differentiation between bacterial and viral infections is critical for timely treatment and complication prevention. Current clinical SAA detection methods include colloidal gold immunochromatography (CGIS), ELISA, immunoturbidimetry, and fluorescent immunoassay (IFA).
