Detailed introduction

Progesterone is an important progestogen, a steroid hormone with a relative molecular weight of 314.5. It is primarily produced by the ovarian corpus luteum and, during pregnancy, by the placenta. It is a precursor to testosterone, estrogens, and adrenal corticosteroids. Progesterone levels are very low in both normal males and females during the follicular phase. After secretion into the blood, it primarily binds to albumin and sex hormone-binding globulin (SHBG) for circulation. Progesterone levels are related to the development and regression of the corpus luteum. The main function of progesterone is to prepare the uterus for implantation of the fertilized egg and to maintain pregnancy. During the follicular phase of the menstrual cycle, progesterone levels are low. After ovulation, progesterone produced by the corpus luteum rises rapidly, reaching a peak concentration of 10 ng/mL to 20 ng/mL 5-7 days after ovulation, transforming the endometrium from a proliferative to a secretory state. If pregnancy does not occur, the corpus luteum regresses during the last 4 days of the menstrual cycle, and progesterone levels decrease. If pregnancy occurs, the corpus luteum does not regress and continues to secrete progesterone, maintaining levels equivalent to the mid-luteal phase until the 6th week of gestation. During pregnancy, the placenta gradually becomes the primary source of progesterone, with concentrations rising from 10 ng/mL - 50 ng/mL in the first trimester to 50 ng/mL - 280 ng/mL in the 7th to 9th months. Clinical studies have shown that progesterone promotes ovulation and maintains normal corpus luteum function in non-pregnant women. Insufficient progesterone production by the corpus luteum may indicate luteal phase deficiency, which is associated with infertility and early miscarriage.
